Aim
This interactive session will enable you to develop the skills to document your work in full consultation and shared collaboration with the person you are supporting. This is a tailored workshop intended for anyone who works with consumers of health services to ensure their care/support is fully informed and influenced by the care recipient. It will enable the healthcare worker to ensure their documentation is wholly reflective of the health care relationship and activities. It will also concentrate on ensuring the notes written are well developed and meet legislative and workplace requirements.

Learning Intentions
Participants will be provided the opportunity to:
• Discuss the concept of ‘patient’s notes’
• Explore the idea of partnership in the health worker/patient or consumer relationship
• Demonstrate collaborative note writing using theoretical case studies
• Investigate potential barriers to collaborative note writing and develop skills to overcome these.
